Plasencia, a licensed medical doctor, is accused of distributing ketamine to Perry and his live-in assistant “outside the usual course of professional practice and without a legitimate medical purpose.”
He was arrested and charged with multiple counts of ketamine distribution and falsifying documents related to the investigation. Plasencia faces up to 10 years in federal prison for each ketamine-related count and up to 20 years for the records falsification counts.
Erik Fleming
Fleming acted as a middleman in the operation, distributing ketamine to Perry’s assistant after obtaining it from Sangha. He has already pleaded guilty to conspiracy to distribute ketamine and distribution of ketamine resulting in death.
According to court documents, Fleming admitted to providing 50 vials of ketamine to Perry’s assistant, half of them just days before Perry’s death.
Perry’s live-in assistant, Iwamasa, admitted to injecting Perry with ketamine on multiple occasions, including on the day of the actor’s death. Iwamasa, who had no formal medical training, allegedly learned how to inject the drug from Dr. Plasencia. He has pleaded guilty to conspiracy to distribute ketamine causing death and faces significant prison time.
Dr. Mark Chavez
Chavez, another licensed medical doctor, admitted in his plea agreement to selling ketamine to Plasencia. He has agreed to plead guilty to one count of conspiracy to distribute ketamine.
THE GRUESOME DETAILS: WHAT HAPPENED ON THE DAY OF PERRY’S DEATH?
Court documents reveal a harrowing account of the events leading up to Matthew Perry’s death. On the morning of October 28, 2023, Perry’s assistant, Kenneth Iwamasa, gave him his first ketamine injection at around 8:30 a.m.
Perry, who was reportedly struggling with severe anxiety and depression, had become increasingly reliant on ketamine in the weeks leading up to his death.
As the day progressed, Iwamasa administered two more injections at Perry’s request. The third injection occurred shortly after Perry had asked his assistant to prepare his hot tub. Iwamasa, having administered the injection, left the house to run errands.
When he returned, he found Perry face down in the hot tub, unresponsive. The subsequent autopsy and investigation confirmed that Perry had overdosed on ketamine, which was the primary cause of his death.

U.S. Attorney Martin Estrada and DEA Administrator Anne Milgram highlighted the exploitation that Perry faced in the final weeks of his life. Estrada pointed out that the defendants were fully aware of Perry’s struggles with addiction and yet chose to exploit him for financial gain.
“These defendants took advantage of Mr. Perry’s addiction issues to enrich themselves,” Estrada said during the press conference. “They knew what they were doing was wrong, they knew what they were doing was risking great danger to Mr. Perry, but they did it anyways.”
Milgram added that Perry had initially sought treatment for anxiety and depression using intravenous ketamine under the supervision of medical professionals. However, when the doctors refused to increase his dosage, Perry turned to street dealers, who supplied him with the dangerous levels of ketamine that eventually led to his death.

Here’s someone with a clear and well-documented opioid addiction. He’s on meds to try to deal with that addiction, and he’s very public about it. And, you know, some people are charging him thousands and thousands of dollars for ketamine and taking advantage of that addiction. It’s really sad.”

Three of the defendants have already pleaded guilty or agreed to plead guilty, while Sangha and Plasencia await their day in court.
